TravelNurseSource is working with MedCadre to find a qualified Case Manager RN in Albany, Georgia, 31701 About The Position Job Title: Registered Nurse (RN) - Case Manager Location: Phoebe Putney Memorial Hospital, Albany, GA 31701 Unit: Case Management (Utilization Review) Shift: Days Starting Date: 10/07/2024 Duration: 17 Weeks Key Responsibilities: Collaborate with healthcare teams to develop and implement effective care plans for patients. Conduct thorough assessments of patients' medical histories and current health status. Manage complex discharge planning and facilitate smooth transitions of care. Monitor and evaluate patient progress, adjusting care plans as necessary. Ensure compliance with all hospital policies and regulatory requirements. Provide education and support to patients and their families regarding care plans and resources. Maintain accurate and timely documentation in the MEDITECH charting system. Qualifications: Must hold a valid Georgia nursing license. Minimum of 5 years of nursing experience, with at least 2 years in a hospital setting. Experience in Utilization Review and Care Transitions is required. Proficient in discharge planning and managing complex cases. Ability to work effectively in various inpatient units (med/surg, ICU, L&D, NICU, Mother/Baby, etc.). Current BLS certification is required. Strong communication and interpersonal skills. Additional Information: Candidates must have a permanent address on their driver's license that is more than 75 miles from Phoebe Putney Memorial Hospital.
