Job Summary Responsible for the overall strategic, operational and execution activities for all programs and departments of the Patient Logistics Command Center and Care Coordination functions. Ensures the ongoing design, planning, and strategic implementation of the components supporting access and integration of care for the patient across the continuum. These components include the collaboration and relationship building with internal leaders, community partners, medical staff, community health care organizations and other key stakeholders involved in the care and wellbeing of the patient. Strategizes and executes throughput opportunities to deliver the right care in the right setting for inpatient, outpatient, community-based, acute and post-acute care. Responsible for the development, delivery and implementation of the PPHS Palliative Care model via internal and external collaboration. Provides oversight of operational functions supporting the overall strategic plan improving the efficiency of patient logistics and integrating palliative care as a natural component of the care continuum. Accountable for global oversight of throughput and length of stay metrics, and strategically interfaces with Senior Leadership to evaluate opportunities for continued improvement. Ensures alignment with organizational system strategies, building on existing initiatives to design future operations around 1) coordination of population health tactics; 2) engaging with consumers in meaningful ways; and 3) supporting clinical care delivery on a pathway toward value based reimbursement. Call Center management, Revenue Cycle management and Utilization Management knowledge (Preferred)
