Job Description:

RN/Nurse Navigator

---The Nurse Navigator functions on a multidisciplinary team as an advocate, interpreter and educator for patients.
---Serves as a point of contact for members of the Coffee Select network to coordinate care of participating patients.
---Coordinates patient care throughout the continuum in collaboration with the multidisciplinary team. Serves as a liaison throughout the facility and in the community regarding services provided for Coffee Select patients and patients with unique diagnoses.
---Serves as a single point of contact for patients, physicians, employers, and caregivers to provide resources and assistance with accessing clinical supportive care services offered within CRMC and CRH Physician Practices

---The evaluation is to assure individual performance, departmental goals and organizational goals are aligned. It is designed to support communication between the manager and the employee. Employee perception of their own performance is very important. To maximize the benefit of this process, both the manager and the employee participate in the evaluation process.

---Needs Improvement - Performance is consistently below requirements/expectations. Immediate improvement is necessary.

o There are shortfalls in meeting the standard, criteria or objective.

o The employee requires close supervision or step-by-step guidance for this task.

B. Education
---Associate of Science in Nursing, required
---Bachelor of Science in Nursing, preferred

C. Licensure
---Active RN licensure in Good Standing with the GA Board of Nursing, required
---Certification from Academy of Oncology Nurse and Patient Navigators, Oncology Nursing Certification Corporation or other nurse navigation organization, preferred

D. Experience
---3-5 years as a RN in an acute-care setting, required
---at least 2 years as RN/ Nurse Navigator, required

E. Interpersonal skills

F. Essential technical/motor skills

G. Essential physical requirements
---Sedentary: Exert up to 10 lb. of force occasionally and/or a minute amount frequently - greater than 75%

---Must possess strong analytical and critical thinking skills
---Knowledge of CRMC's Coffee Select Network
---Must be able to act as a liaison between patients, employers, providers, and other stakeholders
