DescriptionJOB SUMMARYThe Stroke coordinator is responsible for coordinating and implementing services and activities related to the Stroke Center. He/she functions as a liaison between the physicians, nursing staff, allied healthcare professionals, patients and their families. Coordinates and facilitates the care for the stroke patient throughout the continuum. Works closely with physicians and nursing to ensure clinical care path is followed, as appropriate. The stroke coordinator participates in program development as it relates to the stroke center. This includes, but is not limited to staff education, community education, and marketing. Responsible, in coordination with the Director, for planning, developing and implementing stroke center initiatives. Will maintain quality standards for responsive service including data management of key quality indicators. He/she will take a leadership role in development and implementation of outcomes evaluation as guided by regulatory and accreditation standards.
