DescriptionJOB SUMMARYThe Stroke coordinator is responsible for coordinating and implementing services and activities related to the Stroke Center. He/she functions as a liaison between the physicians, nursing staff, allied healthcare professionals, patients and their families. Coordinates and facilitates the care for the stroke patient throughout the continuum. Works closely with physicians and nursing to ensure clinical care path is followed, as appropriate. The stroke coordinator participates in program development as it relates to the stroke center. This includes, but is not limited to staff education, community education, and marketing. Responsible, in coordination with the Director, for planning, developing and implementing stroke center initiatives. Will maintain quality standards for responsive service including data management of key quality indicators. He/she will take a leadership role in development and implementation of outcomes evaluation as guided by regulatory and accreditation standards. All employees are expected to perform their duties in alignment with the vision and values of the organization. The person doing this job is responsible for a variety of tasks that lead to a high level of customer satisfaction in the most cost-effective manner with respect and dignity.GENERAL REQUIREMENTSAdheres to the hospital and departmental attendance and punctuality guidelinesPerforms all job responsibilities in alignment with the core values, mission and vision of the organizationPerforms other duties as required and completes all job functions as per departmental policies and proceduresMaintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs)Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs.
