looking for someone to join our Substance Exposed Newborn/Neonatal
Abstinence Syndrome (SEN/NAS) program into Circuit 2 (Gadsden,
Leon, Liberty, Wakula, Franklin, Jefferson Counties). This program
supports families who are experiencing, or at risk for, SEN/NAS.
CDAC is seeking one Care Coordinator to provide care coordination
services to women and families with substance use concerns in
Circuit 2. The position requires clinical skills and experience
with substance use and mental health disorders, parenting and
family issues, and community resources. Candidate should have a
working knowledge on the impact to families with substance exposed
newborns (SEN) and Neonatal Abstinence Syndrome (NAS). As part of
the SEN Team, the Care Coordinators will coordinate and assist in
addressing all the needs of the families to include substance use,
mental health, medical, financial stability, and overall wellness.
The Care Coordinator will work closely with community providers and
with child welfare providers to advocate and assist in meeting
these families' needs. The candidate must demonstrate an
understanding of ROSC (recovery-oriented systems of care)
principles, case management and care coordination principles, and
have experience working with people with substance use and mental
health concerns. This position will serve all counties (Gadsden,
Leon, Liberty, Wakula, Franklin, Jefferson) of Circuit 2 and serve
counties (Madison and Taylor) of Circuit 3. Bachelor's degree in
social services field with at least two (2) years of relevant
experience required. Master's degree preferred. This position
