Nurse Practitioner Registered Nurse RN The Nurse Practitioner works independently under a collaborative agreement with the Medical Director of the Practice and receives administrative supervision from the Practice Administrator or designee. The function of the Nurse Practitioner is to provide primary health care services to individuals of all ages as a member of a health team assigned to the Practice. The Nurse Practitioner's practice is in accordance with state and local rules and regulations. Assess diagnoses, treats, manages, evaluates and monitors acute and chronic illnesses, administers prevention services, assessment services and other services as designated and ordered by the Physician. Orders, performs, interprets, evaluates, and reviews diagnostic studies and procedures. Prescribes and dispenses pharmaceuticals, evaluates outcomes and dictates changes in management levels of therapeutics. Performs minor surgeries and procedures designated by Practice Physicians. Provides health education and counseling to those clients seeking improvement in vascular health and promotes positive health behaviors and self care skills. Makes referrals to health care professionals, specialists, other clinics and community agencies. Maintains client records in a systematic format approved by the Practice and in compliance with appropriate regulatory requirements. Lifeline Vascular Care works with premier physician practices to develop and manage outpatient centers that provide focused and coordinated vascular access procedures to patients receiving hemodialysis treatment for end-stage renal disease (ESRD). We currently manage more than 60 physician-owned vascular access centers where we provide physician practices with financial and outcomes reporting, staffing and training, facility operations, regulatory compliance, and insurance billing services. The Lifeline network of managed vascular access centers is accredited by the Joint Commission. Minimum Qualifications: Master's Degree in Nursing and relevant Nurse Practitioner Certification Basic Life Support (BLS) Basic computer skills and proficiency in MS Word and Outlook Advanced Cardiac Life Support (ACLS) Certification of a Nurse Practitioner in area of specialty by the ANA and position of license issued by the State as a Nurse Practitioner. Maintain recertification every three years.
