Job Description:
HCA Healthcare is seeking a dedicated Part-Time RN Case Manager
to join our diverse team. In this role, you will be instrumental in
managing patient care, ensuring that optimal healthcare services
are provided in a cost-effective manner. You will work closely with
other healthcare professionals to develop, implement, and assess
personalized patient care plans. This position requires a
compassionate individual with a strong commitment to improving
patient outcomes. Assess, plan, and implement care strategies for
patients. Coordinate with healthcare teams to optimize patient
treatment plans. Monitor patient progress and adjust care plans as
needed. Ensure compliance with healthcare regulations and
standards. Educate patients and families about treatment plans and
healthcare management. Strong clinical skills in a healthcare
setting Excellent communication and interpersonal skills Ability to
make decisions in high-pressure situations Proficient in medical
software and patient management systems Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Valid Registered
Nurse (RN) license in the state of practice Certification in Case
Management (CCM) preferred
